The Flight of MAC249 (CRM Training Video)

This training video was originally manufactured by the United States Air Force. The crew of a C-141 Starlifter are confronted with a partially retracted landing gear after departure from a snow covered runway with a destination of New Zealand. The crew are required to confront and solve a range of problems – documented and otherwise. While not specifically mentioned, the crew implement a decision-making model to guide their decision making process.

The Problem with Personalities (CRM Training Video)

The captain quietly accepts Paul’s vehemence (with a restrained frustration) until he reacts in a way that might potentially alienate the FO from providing future input. Earlier intervention by the captain (affirming his command) in a pleasant manner – and in a way that wouldn’t have compromised their working relationship – might have prevented a future damaging confrontation.

Mission Implausible – A Flight to Calamity (CRM Training Video)

This training video is a little dated (and rather poor quality), but it’s still a useful tool. We’ve made minor edits to the original video (with permission). The video takes a look at a single sector that’s littered with human factor and CRM discussion points.

Groupthink is a psychological phenomenon that occurs within a group of people, in which the desire for harmony or conformity in the group results in an irrational or dysfunctional decision-making outcome. Group members try to minimize conflict and reach a consensus decision without critical evaluation of alternative viewpoints, by actively suppressing dissenting viewpoints, and by isolating themselves from outside influences.

In quite a bizarre incident, two Russian Su-24 fighters buzzed the USS Donald Cook (on the 12th April 2016) in what is described as aggressive manoeuvres. The vessel was operating in the Baltic Sea (International waters) about 70 miles from Kaliningrad.
